APPLICATION GUIDELINES

All applicants should submit the application form and submit the requirements to the Guidance Office. ( see list of requirements below)

The Scholarship Committee will set a schedule for the screening process.

The result of the application for scholarship program will be disseminated or post on the school bulletin board before enrollment.

Successful applicant will be notified thru their contact numbers and will be posted on the School Bulletin Board by the Scholarship Committee.
For Freshman
- Form 138/Reort Card (Original/Photocopy)
- NSO issued Birth Certificate (Original/Photocopy)
- Baranggay Clearance
- Letter of Application addressed to the President thru the Scholarship Committee Chairman
- 2 pcs. 2x2 photo
- Certificate of Good Moral Character from High School Guidance Office
